MEDICAL REQUIREMENTS
Below is a list of medical exams required by the State of California in order to compete.
____ Physical
____ Neurological Exam
____ Blood Lab Work (See accompanied form for tests that are required)
____ MRI accompanied with a MRI summary
____ EKG and EKG Summary
____ Opthamologic Exam (must be completed by an optomologist not an optometrist)

***For blood work it is VERY IMPORTANT to ask specifically for Hepatitis B Surface Antigen, HIV and Hepatitis C-Antibody --THIS IS CRITICAL.
***Medical exams are good for one Calendar year from the day exam where completed.
***Remember if you had an MRI done in 2007 you do NOT need to have one for 2008.
***Blood work expires every six months from the day they were completed.
***Additional copies of Licensing and Medical forms can be found online at www.dca.ca.gov/csac/
Medicals must be completed on these forms.

*** MAKE SURE YOU GET COPIES FOR YOUR RECORDS and FAX THEM TO ME at 949.387.6121 so I can submit them to the California State Athletic Commission
